Philadelphia’s Health Intervention Program for Families (HIP) provides home visiting and other support to families with infants and children (0-21) who have special health care needs.
Children with special health care needs (CSHCN) are children who have or are at increased risk for chronic physical, emotional, behavioral and developmental health conditions. The medical and social needs of CSHCN often require a high degree of support.
